Default Contact Us Error

Hi,

I make a fesh install of Joomla 3.6.4 (with no sample data) and Fabrik 3.5.1.

Install Fabrik sample data. Create menu to Contact Us form. Click 'Add' and page goes to Error 500.

Go to backend, Form, Contact Us, Layout, set PDF icon to 'Hide'. Then go click 'Add' button and error disappear.

Did I miss something obvious here?

I assume the Fabrik sample data is not really up-to-date.

PDF is only working if the Fabrik domPDF library from Fabrik GitHub is included in /libraries/dompdf

Edit:
The problem is not PDF but the form's template: on install it's set to "default" which doesn't exist.
After saving the form once (with or without any modification) this is corrected.

@troester - oops, didn't notice your commit, just merged mine. Sets it to bootstrap rather than leaving it blank.
