Files coming unlinked from display in back end

It seems that we are uploading files and they are somehow going missing. I checked the server and they are there in the folder but on the Fabrik side in the back end nothing is showing as if we never uploaded anything.

The screenshot I'm showing is of a file upload field and the add file button is there but sometimes even that disappears. As you can see there are no files. However when I navigate to where the file is stored for this product it IS in the files on the server.

This is happening randomly.

What do you get in the details view or list view? Are the files (images or filenames, depends on your settings) displayed?

The files and button in form view are displayed via JS, so if they "disappear" it may probably be a JS error. Have a look at your browser console.
