issue with javascript showing/hiding an element

if element is not shown when form first appears, it can't be shown. if it is, it can be hidden and reshown.

if it is not shown on initial form, showing it gives this js error:

The js issue should be fixed (I tested that on your site) - we weren't explicitly including the bootstrap js file, so whilst protostar does include it your warp templates weren't.
That might have fixed the form js show/hide issue as well?

not sure that it's fixed. try this on my site:

1. go to leads
2. click on first lead that is in 'new' status. this causes loss reason element to be hidden as the default when you go in.
3. change the status to 'loss' and you see the space for the loss reason element open up, but no element is displayed.
