Redirect and form success message

I used a redirect plugin in 'app_assessment' form and only set a submission message. But after submitting the form it shows 404 error page, I didn't set any thing in the Jump page!!

I need to return to same page with a clear form and shows the submission message.

If you don't set a jump page, it'll try and use whatever the HTTP_REFERER page is, which might be blank. If you want to go back to the same page, explicitly add a jump back to that page, with a relative URL ... index.php?option=com_fabri&view=form&formid=123&Itemid=456
