using the Calendar popup on custom form

I'm sure it's simple. I've created a custom form. How do I get the calendar popup icon to appear next to my date field? I've seen other Fabrik sample forms using it and read that it uses Joomla's calendar bot. Is there some script to insert manually? Thanks.

not sure exactly what you mean, could you post some example pages where the popup icon appears and your form where it doesnt?
