Is your intention to send the confirmation to the person completing the form? If so, be sure the word email is {email} so it picks up the person's email address.

Thanks for responding, Andre. Unfortunately, the labels are quite verbose for viewer purposes, and the values are more succinct. I'm pretty new to the db thing and am not sure what you mean by lookup tables. How are those different from viewing the Fabrik tables?

I'm noticing that radio button data is different when displayed in the Fabrik table (table shows the radio button label contents) than it is when the table info is displayed in Navicat (Navicat shows radio button value contents). Why is this and how can I force Fabrik table to list radio button...

How do I get a multiple radio button element to display each option on its own line? It tries to pass them all on one line and it makes it very hard to read.
