need to have a background image on a form

I need to have a form like :
http://www.bachpanglobal.com/fe.html

This form is fabrik2 on joomla 2.5.
I need for fabrik 3 on joomla 3.6

In that form the following css:
.new-fanch-form{background: url("http://bachpanglobal.com/images/franchise_enq.jpg")}
.new-fanch-form .fabrikinput.inputbox.text, .new-fanch-form .fabrikinput.inputbox, .new-fanch-form .inputbox.text {
width: 230px;
}

was used to get the background image.

I tried using this css on the new version, but it did not work.

Please suggest as it is very important for the client.

Use a browser dev tool (Chrome, Firefox, firebug on firefox...) to determine the CSS class or id you need.

Quiet a layman in such css.
Tried to do some research & development, but no success.
What should be replaced with .new-fanch-form - the page alias or form alias .

I have added some css code to the template.css. The background image is showing, but teh form is out from the defined fields.
The page source and css code is attached.
