form destroys the menu on the page

On this link, the left menu links and menu links at the bottom position have vanished, where as the same are visible on other pages.

Please suggest.

You had this issue with your Joomla template and custom CSS in some other thread (as far as I can remember something was set to hidden).

Fabrik is working without issues with the bootstrap (BS2) protostar template coming with Joomla.
If you want to use an other template not compatible with the Joomla one this is no Fabrik issue.

I am using a template (mx117) for joomla3 .It is from mixwebtemplates.com .
On another form, everything is working fine on the same template.

I don't know why I'm answering at all...

It's not even your non-standard Joomla template but your own custom Fabrik templates and custom_css.php settings.
If you are setting .nav {display:none} it won't display!
Please don't ask stuff you've asked before in other threads and where you are responsible yourself.
