Form does not load in module after latest GitHub update

I have a fabrik form module on this page http://pieterdebuysser.com/nl/data.html.

On load, the module is hidden by CSS (div#middleRightColumn {display: none;}) so it can be shown via JS when you click on links in the main content of the page.

This used to work until this morning's update but now I have JS error saying values are empty.
After researching, the reason these variables are empty is that the form does not load and I don't know why.

It should load a custom template, but even with the default one, the problem is the same.

Joomla 2.5.14 + latest GtHub (master branch)

I don't think this is the form not loading as my test form in a module loads fine.
It also loads the assigned template without issue as well.

Simply question have you tried saving the module, ensuring that the options are all filled in?
