adds not bringing in proper id

an add on this page:

is a link to this page:
http://www.skyrun.com/skyforce/leads/form/4/?sf_activity___lead_id_raw=1

however the lead_id is not included on the form (nor in the database after the save).

(same thing on the next level down... click on any of the 'scoring' links which brings you to a page like:
http://www.skyrun.com/skyforce/lead...5=0&resetfilters=1&fabrik_incsessionfilters=0

then click add and the added record doesn't have the activity_id included.
