approvals visualization not working

If you get a wsod then you probably need to turn on PHP error reporting and increase its reporting level - thats a separate thing from turning on Joomla's debug option.
Seems this viz never got any Joomla3 love - I've made a very initial attempt to get it working https://github.com/Fabrik/fabrik/commit/893a7e6df88d1a2c9e5b083b0f0fecc45b9e166e. It probably needs more work on it though before it works really well, the whole thing was coded for a specific client with specific needs, which may not make it that flexible a solution at the moment. e.g. you are limited to 5 approvals, and there's no pagination.
