Cascading Dropdown for Countries

I have been working on this cascading thing now for a bit and I think I got it all figured out, however I have a few questions. Let me give you a little background as to my setup..

I used Geo World Map to build my tables. Found here:

I currently have three tables with records. Very comprehensive. I love it so far.

countries
regions
cities

I have three elements (for now) that I am working with in cascade:

country (databasejoin to countries table)
regions (cascade, watches country)
cities (cascade, watches regions)

Im able to see the cascading effects just fine. Thats not the problem. My problem is that some "regions" don't have cities.

For instance, in my setup, if you select "Afghanistan" as the country, then "Kandahar" as the region, nothing appears for "cities". But if I try, say, any region in United States (but itself, see below), such as "Alaska", it shows all the cities in Alaska.

There is also a peculiarity with how countries are also listed as regions. Try selecting "United States" as your country, then select "United States" as your region:
