Element In Repeat Group Form Tab Not Shown

Hi , wonder if anybody can advise what went wrong. I had 2 tables first is the employee details table and 2nd table which is declaration record make repeatable mean employee can enter multiple record. I want to display it in Tab form but the element in the second tab somehow did not appear or rendered properly.
