Form opening in View Mode rather than Edit Mode

Hi,

I have a form for public and registered users. All was working fine, but now when the form opens, it is in view mode when the user is not logged in. I first saw this yesterday in Safari only, but now its all browsers. I know, it sounds like I've changed something... maybe, but all I can think what... I installed Advanced Module Manager but I have disabled it for testing.

The only way I've been able to work around this is the following:
In the menu item which loads the form, enter a very big, non-existent row id to load.
This indicates to me that its not settings on elements etc.
 
On my PC your form is in view mode with FF, Chrome and IE9 (all looking the same, no labels-above).

Loading a form with no (or a not existing) rowid should open a new record.
What are your access settings (in your list) for view/edit/add records?
What is your primary key? It must be integer, autoincrement.

I did not realise that access settings in the List view affect the form.

And re showing correctly in FF for me... I was logged in, so had access (red face)
