hmm this looks to be special to these docs. something wrong with joind data and relation. joined data not deleting. ? running tests now.. to figure it out..
this duplication does not happen with other doctors with the same last name..

yep. that is what it was. extra joined data or something.. I enabled delete joined data. and removed all the relations.. and created new ones.. that issue is gone now
