If you tap on any of the fields it does not bring up keyboard it just looks like it is an image of a form...nothing is fillable or clickable

It's a problem with your Joomla template which doesn't seem to be really responsive.
You can see the same effect if displaying your desktop browser window in a small (tablet/phone) size.
Your <div class="rt-grid-6 rt-omega"> is overlaying <div class="rt-grid-6 rt-alpha"> which is containing the fabrik form, so preventing any input.
