I am trying to recreate this form in joomla and I am really confused.

I created the first package but how do i Create a second package? Do i need to redo the first step when i created the first package? I tried but the radio button was selected as default.

What is the easiest step for me?

The trick to get rid of the default selection of the radio button:
add a second option (sub-element) then delete the "default" one.
