Cannot sort or add/remove groups on a form (3.0)

Using 3.0 and I'm unable to adjust the current groups list when I edit a new form.
This is the 3rd or 4th time I've tried creating a form and have had this issue come up.

I cannot shift them up/down or add/remove groups.
