Custom search form in Joomla category

Hello friends

In my site i have a category for announcements and i need two extra fields for the users who will be used like a search form. The first field for the years (select a year) and the second field for the months (select a month). I can do this with custom fields from Joomla but i don't know how i can to include it in a fabrik list or form.

So, you want to search/filter a list?
Why don't you make it filters then?
For list settings: http://fabrikar.com/forums/index.php?wiki/list-filters/
To set elements' filters up, edit them, then -> List view settings -> Filters.
