form apply button directs to site main template instead of cassiopeia

I have a gantry5 customized template for my main joomla site and i use cassiopeia template with menu links for my fabrik lists. When i click the apply button in my forms it used to work fine with fabrik3. However now with fabrik4, after clicking the apply button it displays the form data with main site template. In case it helps i have noted the following issues:

1. if i enable Search Engine Friendly URLs:
with fabrik3, apply button goes to https://mysite.com/mymenulinklist/form/1/20782
with fabrik4, apply button goes to https://mysite.com/component/fabrik/form/1/20782

2. if i disable Search Engine Friendly URLs:

with fabrik3, apply button goes to
https://mysite.com/index.php?option=com_fabrik&view=form&Itemid=405&formid=19&rowid=1&listid=19

with fabrik4, apply button goes to
https://mysite.com/index.php?option=com_fabrik&view=form&formid=19&rowid=1&listid=19
