Form NOT writing to DATABASE (list)

Hi,

I have a strange error. When users submit a form (in frontend) it doesn't "send" data to fabrik database / list!

Are there any know issues about this problem? It can be related to use of Cookies on website?

All elements are set to: "show in list". "Record to database" is also set to "YES" in "EDIT FORM -> FORM PROCESING"

System:
- Joomla 2.5.14
- Fabrik 3.0.8.
- Php 5.3.21
