Getting Message: "Form not Published", even though it is published

Created my first simple list & form.
In the backend, it is shown as puclished.
I assign this form to a menu item.
I go to front end, and choose this menu item.

Instead of the form, I see a message that the form is not published. But it is....

What could be wrong?
