Place Holders not working

My place holders on links, and email are not working.

They worked fine when I first built them but now that I have updated from git hub a couple of time they dont seem to work.

However they are working fine on autofill plugins.

Place holders work in the back end

Rob this part of the form is a join from another form which is located at http://www.ems-complete.com/ccems/index.php/riskassessment/form/13/

While doing some testing today I noticed that the place holder work on this form but not on form #3

I am at a complete loss here...

I don't seem to have a backend login any more. I tried the two logins that LastPass remembered me using for your site the last time I worked on it, but neither of them work any more.
