Really Need some help - everything is broken

Help - it seems that I have somehow broken my Fabrik form badly. I need some help.

Everything was working but Now my form does not display any fields.

1. I updated to the latest from GitHub.
2. I added a couple of fields.
3. Changed a few labels.
4. I tried to get a CascadignDropDown to work. but could not. so I trashed it.
5. I added some new CSS, .folderselect-container .breadcrumbs {display:none;}, but removed it and it is still broken.

I have also noticed that a bunch of Language Strings are broken.

I would really appreciate any insight. All of my site details are in my profile. The front end of the site is at http://sendlater.savior<REMOVE>labs.com
