button element to show group of elements when clicked

if i have a form and created a button element "book" on this form and need when clicked this button to pop up a group of elements , i made a javascript in this button element when click , what the settings i do else ? to popup this group of elemets ( customer name, phone no ) then need to submit this data ??

That's not something we can really help with on a subscription. Subscription support is really for assisting you with built in features of Fabrik, not for writing custom code. We obviously help with small amounts of simple code where we can, but what you are needing to do in this case would not be simple.

you cant just give me the starting point i can start from ? maybe a javascript simple code to put it the button element ?

No, because I can't even think of a good (or even possible) way of doing this. If I spent an hour playing around I might be able to come up with something. Popups which submit data are not simple things to code.
