Error 500 Layout default not found

i created module and display in an article using load module.the list appear but when click on Add , it will say 500 Layout default not found
 
As per my replies in email, I'll need you to set the account you gave me to Super Admin, so I can enable debugging.

@burq - can you add your site details in your My Sites list (look to the left of any of your posts). For now we just need a super amdin login and a backend URL, we don't need ftp or phpMyAdmin details.

Notes for other support folk ...

Hitting the Add button get a "layout default not found". Thought it might be the template, or that he's using a list in a module position (in an article using some kind of modules anywhere thing) ... but even if I use a straight up component link ...

As I suspect it's to do with another plugin setting the layout parameter, I'm going to have to disable a lot of your modules and plugins, and re-enable them one by one. is it OK to do this, i.e. is anyone using this site?

It's ok. I managed to create a new list and form. The new form is working. You can click on eTempah link to view. Maybe something wrong with your sample form.
 
Hmmm, interesting. I haven't tried using the sample one for ages, I'll have to try doing a fresh install here and test it.
